Our packages range from services that meet the needs of the children to the older person, those with learning or physical disabilities or people that may need assistance at home with their activities of daily living due to their situation and condition. We aim to provide nursing care to service users that have reached the end of life and have chosen to remain in their own homes for this. We will liaise with the NHS to ensure that personal and nursing care is delivered, centred around the needs of our service user. Our services will be offered to the whole population, including people that have life threatening diseases, mental health as well as physical health needs. We will also provide care and support to children and their carers. We will also provide services outside the home of the service user to ensure independence and well-being. We offer various packages and services that give carers respite from their caring responsibilities.
Our aim is to work hand in hand with the service users and their carers to provide individualised care that has been assessed and agreed. At DCW we aim to deliver service user centred care through the provision of a quality service. Whether the service user wants a continuous package of personal care, an occasional visit for companionship or household cleaning, DCW will match these needs with our well trained and skilled care support workers. Our aim is to provide a consistent, appropriate and flexible care that is customised according to individual needs.
Over time we intend to offer our service users a variety of services including therapeutic support groups for people with mental health needs, drug or alcohol issues. We aim to provide groups for these service users to offer them support and social networks in which they are able to share their experiences in a therapeutic setting. DCW will offer these services in areas that are easily accessible to individuals after consultation with commissioners and service users
